The Innovation Opportunity: Rethinking Program Management for Modern Care
Healthcare organizations today manage multiple, specialized programs—chronic disease management, behavioral health, substance abuse treatment, and population health initiatives—all at once. Each program brings unique workflows, compliance requirements, and performance measures. Legacy systems often isolate these processes, slowing operations and increasing risk.
We saw an opportunity to design a unified, intelligent, and adaptive solution—one that automates the complexity, integrates value-based care requirements, and delivers real-time insights to all stakeholders.
The Breakthrough: A Comprehensive, Future-Ready Program Management Engine
Our Program Management Engine, which we developed approximately five years ago, combines automation, intelligent workflow orchestration, and performance analytics into a single, integrated solution.
Key innovations include:
- Multi-level Enrollment Workflows – These workflows feature outcome-focused approval stages to guide patients through complex journeys seamlessly.
- Dynamic Care Team Assignments – A system that assigns care teams based on patient needs and risk stratification, with system visibility providing clear accountability tracking for each care provider at every stage. This ensures seamless transitions and transparent responsibility handoffs throughout the patient journey.
- Intelligent Workflow Automation – Automated status transitions in the workbench based on configurable VBC-aligned rules, triggering quality metric tracking and assigning care tasks while proactively flagging potential care gaps.
- Dynamic Program Versioning – Version-controlled program updates with effective dates, preserving historical accuracy for reporting and compliance while enabling agile adaptation.
- Advanced Revenue Cycle Integration – Unified handling of multiple payment models, from fee-for-service to bundled payments and shared savings, with selective CPT rollups and grant-based funding.
- Real-Time Dashboards & Trackerboard Integration – Configurable dashboards for scheduling, queue management, and population health monitoring—giving stakeholders instant insight into both operational and VBC metrics.
- Population Health & Risk Stratification – Sophisticated patient classification tools for identifying high-risk cases, tracking care gaps, and implementing targeted interventions, including ASAM scoring for addiction programs.
From day one, our approach was built on innovation principles:
